What is azelaic acid?
Azelaic acid is a dicarboxylic acid found naturally in grains such as wheat, rye, and barley, and also produced by yeast that lives naturally on human skin. At 15–20% concentration (available by prescription in some markets), it has comparable efficacy to topical antibiotics and benzoyl peroxide for acne. At 10–15% (widely available in over-the-counter formulations), it is highly effective for pigmentation, redness, and rosacea.
How azelaic acid works
Azelaic acid works through multiple pathways simultaneously. It is antibacterial, inhibiting the growth of Cutibacterium acnes (the bacteria associated with acne breakouts). It is also comedolytic — it normalises the abnormal keratinisation inside pores that leads to blackheads and whiteheads. At the same time, it reduces inflammation, calming the redness and swelling associated with both acne and rosacea.
For pigmentation, azelaic acid is a potent tyrosinase inhibitor — it blocks melanin production at the source. Crucially, it targets only the hyperactive melanocytes responsible for dark spots, leaving normal pigmentation cells unaffected. This makes it uniquely safe for darker skin tones, where some brightening ingredients (particularly high-strength hydroquinone) can cause uneven lightening.
Who should use azelaic acid
Azelaic acid is particularly well-suited to patients who cannot tolerate stronger actives — those with sensitive or reactive skin, rosacea, or Fitzpatrick Type IV–VI (olive to dark) skin where irritation from AHAs or retinol would risk post-inflammatory hyperpigmentation. It is also one of the very few active ingredients confirmed safe during pregnancy and breastfeeding, making it an important option for expectant mothers who want to continue treating acne or pigmentation.
For adult hormonal acne — common in women in their 30s and 40s — azelaic acid works particularly well as a standalone treatment or as part of a routine alongside niacinamide. It has a slower onset than retinol or AHAs but a much lower side effect profile.
